When I open Firefox the entire menu bar is transparent. There are no close, minimize or maximise buttons. The lower menu bar is like wise transparent. I have uninstalled and reinstalled with the same result.

This is likely to be caused by an incompatible/malfunctioning theme or add-on. Try the procedures in the Troubleshoot extensions, themes and hardware acceleration issues to solve common Firefox problems article.
