The address toolbar is missing. I have followed the online guides to resolve this issue- not working. Any other ideas?

The address toolbar is missing. I have followed the online guides to resolve this issue including disabling add-ins and extensions. The problem does not occur in safe mode. But does occur even when acceleration, all extensions & add-ins and themes are disabled. I have also reloaded firefox 4. Any other ideas?

Please do the following:

  1. Click Help | Restart with Add-ons disabled.

  2. In the next menu which appears, checkmark: "Reset toolbars and Controls".

  3. Finally, click the button called "Make changes and restart".

If the problem doesn't occur in Safe Mode, restart again in that configuration and then in the menu which appears, checkmark the option: "Disable All Add-ons". Then click the button called "Make changes and restart".

Next, re-enable your add-ons one-by-one until the problem recurs. Then you'll know which one is the culprit.

All my add-ons are disabled but as soon as I restart outside of safe mode (eg. restart after safe mode with all add-ons disabled, in fact I have deleted all of them) the problem reoccurs. Bizarre!
