Why does firefox 15.0 sometimes ignore proxy and attempt direct connection?

Since updating to Firefox 15.0 on my Win XP SP3 system, the firewall is sometimes reporting that Firefox is attempting to connect directly to web sites. Note, all requests should go via proxy as Firefox is configured with a local .pac file that directs requests to the proxy server. The configuration is same as used with 14 and earlier versions. Most http/https requests are directed to the proxy server. The problem is that some requests are made directly to the http or https servers which is noticable as the firewall raises an alert for each blocked request.

Start Firefox in Safe Mode to check if one of the extensions or if hardware acceleration is causing the problem (switch to the DEFAULT theme: Firefox/Tools > Add-ons > Appearance/Themes).
