I cannot get the import file function in address book to work.

I have csv files from my gmail account and from Outlook 2003 but having spent hours trying to map the fields I eventually settled for a very limited subset of the contact details: each contact displays when I select 'Next' but when I click 'OK' it adds a sub-address book, always with 0 contacts.

I have waded through many past forum posts and tried to export a Vcard file from Outlook 2003 (it does not give the option); I have removed most of the columns of the csv files and moved them about to avoid the horrible way that the 'Move Up' and 'Move down' buttons shift other, previously reset, fields - apparently at random; and even if I limit the import to just name and primary email address the import program always produces a sub-address book with zero contacts.

Is there a proper, working program to enable the migration of contacts from outside Thunderbird?

For your Gmail contacts. A couple of things you might have missed.

When exporting Gmail contacts, choose "Outlook CSV format (for importing into Outlook or another application)" How to Export Your Gmail Contacts

When importing the CSV into Thunderbird, make sure First record contains field names is checked. How to Import Your Gmail Contacts into Mozilla Thunderbird

Same goes for importing the CSV from Outlook, make sure First record contains field names is checked.

If Outlook is installed on the same computer as the computer you are importing the contacts into Thunderbird, then refer to this: Thunderbird - Importing Outlook Emails and Address books
