How do I remove a User ID and Password stored in Firefox running on a Mac?

I apparently saved a user id and password when I signed onto an application that I opened on a Mac in Firefox 3.6.6. As a result, anytime I access that app via Firefox, the User ID and Password are filled in.

I would like to delete that particular user id and password but can't find how to do it. I have Cleared Recent History > Form & Search History and Cache and Active Logins. But nothing worked.
