Are you using an Addon or Theme which affects the set of buttons? If yes disable it to see if you still having problems. Otherwise try View > Toolbars > Customize > Restore default set. Hope this helps.

Thanx for the prompt reply. I did use Silvermel (very nice!) 1.4.4, bit disabled it when the problem occurred. I also Restored and customized after I had disabled Silvermel, but the problem persisted. When I use Tabs on Top or change the tabs back to below the Navigation Bar, the Reload button outside the address box disappears, but when I open Ff again, the restore button is back. Strange... I don't use the Bookmarks Toolbar (I have a bookmarks button on the Navigation Toolbar in between the Back/Forward buttons and the address box - my Forward button is still there, altho not active) or Menu Bar. I also use the Add-on Bar at the bottom.

Below a pic with the extra Reload button next to the address box. The icon/button in between the address box and the extra Reload button is Trusteer Rapport for my Internet banking). Ah! Maybe this is the problem. When I hide the Trusteer icon/button, the Reload button also disappears. When I show the Trusteer icon again, the Reload button appears again.

2nd image has no Stop button and has 2 Reload buttons, plus you have something I can't identify immediately to the right of the Location Bar.

Open the Firefox Safe mode and select Reset toolbars and controls, then hit Make Changes and Restart. Re-do your toolbar customization from scratch.

The button in between the two Reload buttons is the Trusteer Rapport button (which my bank suggested using with Internet banking). With other "browsers" this Trusteer button is also inside the address box (with my Ff it's just to the right of the address box by default). It appears that this button is the problem. When I remove it, the Reload doesn't duplicate - when I put the Trusteer button back, the Reload button duplicates again (even after a complete reset and customization of buttons). The Trusteer button also brightens when entering a secure site. I had a look inside the new localstore.rdf and from what I can decipher, it looks fine. What I also noticed, is that the second Reload button disappears when I toggle to Tabs on Top. However, after a restart the duplicate Reload is back again when the Trusteer button is present. I will take this up with Trusteer. Thanx for all the help.
