I recently got a new phone so I disabled 2FA (Microsoft app) so I could activate it again on my new phone. Now I can't activate it because with either Microsoft or Google Authenticator, when I scan the QR code or type the manual code, the 2FA code displayed in the app gets rejected.

Same problem on both phones, and I have since reset the old phone.

Note that in the Google Authenticator you can transfer the 2FA code from one device to another device via the three dot menu in the app, so as long as you have access to both devices then there is no nee to disable 2FA.

What errors do you get if the scan the QR to check whether the TOTP code generated by the app is working ? You can optionally scan the QR with a barcode reader and open the otpauth:// link in the app. Make sure that the time on the device and in the app is correct within 30 seconds.
